Register using your site-specific Tax ID Number (not the physician-specific Tax ID).

Please submit one entry per registry participating site.

NOTE: Be careful to enter the correct TIN (Tax ID Number) for the practice/entity, as this is what CMS uses to identify your site and apply credit for your Quality Score. It will be your responsibility to record this number accurately. CMS cannot process the reported Quality Measures if inaccurately posted.

Monthly fees are charged to the lab and are the same regardless of the number of physician/providers who are subscribers/participants in the registry. The *cost of registry participation per individual lab is $175/month for **OEIS members and $295/month for nonmembers.

*Cost for optional submit MIPS submission to CMS is $399 per NPI billed annually.
**OEIS Membership can be in the Active or Associate Member categories. All enrolled physicians must be members to qualify for OEIS member rate.
